I．はじめに

　central pontine myelinolysis（CPM）は，1959年Adamsら^1）が発表して以来，橋底部にみられる暁界明瞭な左右対称性の，中心性脱髄で，神経細胞，軸索は保たれ，血管異常，炎症などの所見を欠く特異な病変とされている．従来，その成因として慢性アルコール中毒，栄養障害，肝障害などが重視されていたが，最近低Na血症，特にその急速補正によるものが注目されている．また，その診断は剖検によることが多かったが，ComputerizedTomography（CT），聴性脳幹反応（BSAR）により生前に診断された報告も散見される．今回，われわれは臨床経過，CT,BSARによってCPMと診断し得た症例を経験したので文献的考察を加え報告する．

A case of central pontine myelinolysis (CPM) fol-lowing rapid correction of hyponatremia is reported and a review of the literature is made.

The patient was a 63-year-old non-alcoholic female who had no liver or kidney diseases in her past history.

She was found unconscious after a series of con-vulsions and was admitted to the hospital. Eighteen months prior to admission, she had a surgery for a ruptured anterior communicating artery aneurysm. Her postoperative course was uneventful except for an urinary incontinence and mild disorientation. She was initially lethergic with conjugate deviation toward right.
