I．はじめに

　syringomyeliaは，脊随に空洞形成をきたす進行性の疾患群の総称であり，以前は変性疾患の1つと考えられていたが，1958年，Gardnerがsyringomyeliaの病因としてhyd—rodynamic theoryを発表して以来，この疾患の病態の解明，さらには診断，治療の面で非常な進歩がみられるようになった．しかし，その病態についてはなお不明な点も多い．本邦では，この疾患に対する関心度が低いためか，またはsyringomyeliaをきたすArnold-Chiari I型奇形の頻度が欧米に比べ低いためか，本疾患の症例の報告は欧米に比べかなり低いものである．今回われわれは，水頭症に合併した水髄症の1例を経験したので，その病態診断，治療につき若干の文献的考察を加え報告する．

A case of hydromyelia (syringomyelia) combined with hydrocephalus is presented. The patient was a 17-year-old-boy. He was a admitted to our hospital because of disturbance of fine movement of the right hand. Neurological examinations showed right he-miparesis, right upper muscle atrophy and sensor) dissociation of C1-T2 dermatomes. This dissociated sensory impairment gradually deteriorated. CT scan performed 3 and 6 hours after the intrathecal injec-tion of metrizamide revealed an intramedullary syrinx extending between Cl and T4 vertebral levels and communicating with the 4th ventricle.
