四丘体部くも膜嚢胞—5症例の臨床的考察

Arachnoid Cyst of The Quadrigeminal Cistern

I．はじめに

　くも膜嚢胞は良性の非腫瘍性脳実質外占拠性病変で，その内容液として脳脊髄液様の液体を貯溜した嚢胞で，くも膜の存在するあらゆる部位に発生可能であり，その主要発生部位として，sylvian fissure^2,6），interhemi-spheric fissure^13），supra-sellar region^{10,16,17,20,22）}，cere-bello-pontine angle^2,6），quadrigeminal cistern^8,9,14），retrocerebellar region^15），convexity^2,6）などがあげられるが，このなかでsylvian fssureに接して生じる中頭蓋窩くも膜嚢胞が最も頻度が高い^7,18）．くも膜嚢胞はCTの出現以来，その特徴的な所見からCTにより比較的容易に診断することができ，このくも膜嚢胞に関する報告例が増加してきたが，今まで極めて報告の少ないquadrigeminal cisternに発生したと考えられるくも膜嚢胞5例を経験したので，その5例の臨床所見および外科的治療法を中心に若干の文献的考察を加えて報告する．

Arachnoid cyst located near the quadrigeminal cistern has been reported as paracollicular arachnoid cyst, arachnoid cyst of the quadrigeminal cistern, paramesencephalic arachnoid cyst, arachnoid cyst posterior to the third ventricle, cyst of the cisterna ambiens.

The purpose of this paper is to present the clinical pictures, diagnostic studies and treatment of 5 cases of the arachnoid cyst of the quadrigeminal cistern.
