成人で発症した頭蓋底脳髄膜瘤の1治験例—臨床面からの新分類

Basal Encephalomeningocele Occurring in Adulthood: A case report and clinico-anatomical classification

I．はじめに

　頭蓋底の脳髄膜瘤（basal encephaiomeningoceles）は稀なもので，その頻度は35,000の出生毎に1例程度と考えられている^3,14,21）．

　われわれは最近，慢性腎不全に対する血液透析後に髄液鼻漏と髄膜炎をくり返した成人の1症例を経験した．また，本症例はSuwanweiaの分類に従えばtranseth-moidal typeであろうと考えられたが，広汎な骨欠損を有するため，明確かつ細かい分類は困難であった．症状および治療の面から前頭前額部と頭蓋底の脳髄膜瘤を，簡単に，①anterior type，②intermediate type，③posterior typeの3型に分類することを提唱し，さらに本症例における脳髄膜瘤や髄液鼻漏の発生機序について考察を加える．

A 53-year-old male was admitted with complaintsof the recurrent cerebrospinal fluid rhinorrhea andmeningitis. Basal encephalomeningocele was re-vealed by radionucleoid cisternography, skull x-raystudies, and CT scan. Then, it was confirmed byoperation. In reviewing the literature, we proposed anew classification of fronto-basal encephalomenin-gocele from clinical standpoints. 1) Anterior type;detectable facial anomalies, hypertelorism, buldingfrontal mass or masses. Reparative operation is easy.
