頭部外傷急性期における凝固線溶能の特徴—重症度と予後の相関において

Disturbance of The Coagulatory System in Patients with Head Injury in Acute Phase

I．はじめに

　頭部外傷後の凝固線溶異常に関しては種々の検討がなされており，その急性期においては凝固亢進状態にあり，disseminated intravascular coagulation（以下DICと略す）へ移行しやすいと考えられている．またDICの所見を呈するものは脳損傷の程度が強く，予後も悪いといわれている^{3,5,6,9,15）}．これらのことから，今回われわれは単独頭部外傷100例の凝固線溶能について生存群と死亡群の比較検討，脳実質損傷群と非損傷群の比較検討を行うことにより，頭部外傷急性期における凝固線溶異常について重症度と予後の面より検討を加えたので報告する，また，頭部外傷に起因したDICの特徴についても言及したい．

The coagulation and fibrinolytic function of 100cases (male 75, female 25) of trauma limited to thehead wa's evaluated in acute phase. The coagulationand fibrinolytic function was evaluated by 6 para-meters, consisting of platelet counts, PT, APTT,fibrinogen, serum FDP, and protamine sulfate test.Head injury was categorized into 5 groups accordingto CT findings: subdural hematoma (SDH: 13 cases),cerebr 1 contusion (CC: 46 cases), epidural hematoma(EDH: 21 cases), skull fracture only (Fr : 14 cases),and cerebral concussion (Co : 6 cases).
