くも膜下出血再破裂発作の意義—特に早期再破裂について

Rebleeding Attack of The Cerebral Aneurysm:Clinical significance of the early aneurysmal rebleeding

I．はじめに

　破裂脳動脈瘤に対する早期手術の目的は，再破裂の予防とくも膜下出血に伴う種々の病態に対し，できるだけ早期に対処あるいは予防的処置を講ずることであり，手術時期の検討を行う際には患者の重症度，脳内血腫（Ich）の有無，脳血管攣縮（Vs）の有無，脳循環動態といった種々の病態に重点をおいて論じられることが多い^3,11,12），しかし，再発作については長期followの結果に主眼をおいて，その時期や重症度が論じられることが多く^6,10，¹⁴，再発作の予防のためにはどの程度の手術の緊急度が必要かについては現在まで詳細には検討がなされていない．

　今回これらを明らかにするために，くも膜下出血後急性期に入院した症例を対象とし，出血発作の時期，頻度，重症度，予後，手術による救命の可能性，さらにはIch, Vsといったくも膜下出血急性期病態に対する影響についても論ずる．

When one mentioned about aneurysmal rebleeding,timing and incidence of the rebleeding attack (RA)have been mainly discussed in relation to long termprognosis. But it was rarely considered about neces-sity of emergency treatment based on timing and inci-dence of RA.

In this study, clinical significance of RA especiallyin the early stage is discussed in relation to severity ofthe patient's condition, timing of surgery, clinicalresults and so on.

Three hundred sixty-eight cases of anterior com-municating, internal carotid and middle cerebralaneurysms who were admitted on our hospital within7 days after last attack were studied.
