I．はじめに

　脳神経疾患の診断における脳血管造影の重要性については述べるまでもないが，最近digital subtraction an-giography（DSA）が開発され，造影剤の静注によっても脳血管の造影が可能となり，比較的non-invasiveなため主としてスクリーニング検査として使用されるようになった^2,3,7-9）．しかし静注法によるDSAの解像度は特に頭蓋内血管の診断に関しては不十分であり，術前の精査目的にこれを用いるのはいまだ不適当と考えられる．今回，脳神経疾患の診断におけるDSAの可能性と有用性を検討するため動注法によりDSAを行い良好な結果を得たので報告する．

Three hundred and fifty-seven digital subtraction angiography (DSA) were performed in 184 neurosur-gical patients by intraarterial injection. Examinations consisted of 192 carotid angiography, 110 vertebral angiography, 23 aortography, 11 spinal angiography and 21 other angiography. In all examinations, visualiza-tion of the vessels was excellent and the complications were never experienced.

High contrast sensitivity of DSA resulted in better visualization of tumor stains, phlebogram, and arteries in cerebral arteriovenous malformations with large shunt blood flow than conventional angiography.
