半球間のう胞を伴った脳梁欠損症の1治験例

Agenesis of the Corpus Callosum Associated with Interhemispheric Cyst:A case report

I．はじめに

　脳梁欠損症は単独では無症状のことが多いが，合併する種々の奇形^8）（水頭症，孔脳症，agyria,microgyria，　Arnold-Chiari奇形，Dandy Walker奇形など）により脳外科的治療の対象となることがある．今回われわれはけいれん発作を主訴として入院した，稀な半球間のう胞を伴った脳梁欠損症を経験した．本症例では，特異なCT所見が得られ，術前に髄液循環動態を把握した上で，のう胞腹腔シャントを施行し，良好な結果を得たので若干の考察を加え報告する．

A 7-year-old boy was admitted because of convulsive seizures. He was diagnosed as agenesis of corpus callosum associated with interhemispheric cyst by CT scan and angiography. Metrizamide CT cisternogra-phy and cystography disclosed no communication between interhemispheric cyst and the third ventricle. By means of metrizamide CT cisternography and cystography, the authors considered this interhemi-spheric cyst gradually increased by some kinds of ball-valve mechanism.

Cysto-peritoneal shunt was carried out.
