小さな硬膜欠損により生じた術後性脊髄硬膜外くも膜のう腫

Postoperative Lumbar Extradural Arachnoid Cyst:Report of two cases and review of the literature

I．はじめに

　腰椎椎間板ヘルニア摘出術には，さまざまな合併症があり，腹部大動脈や尿管の損傷のほかに，Ford^5）は手術局所の合併症を詳細に報告している．

　われわれは，術後数ヵ月を経過し，いったん順調に社会復帰したのち，再び強い腰痛が出現した2症例を経験した．この2人を再入院させ精査したところ，手術部位に一致してくも膜下腔の後方への異常拡大を認めた．

　再手術の結果，初回手術時の小さな硬膜欠損が原因で生じた硬膜外くも膜のう腫であることを，組織学的にも確認した．これに類似した報告は，われわれの調べ得たかぎりではGrumme8）のもののほか意外に少ない．そこでくも膜のう腫の成因について文献的に調査した結果，腰部の手術では，小さな硬膜欠損の果す役割が非常に大であることがわかった．

Two cases of postoperative extradural arachnoidcyst in the lumbar region were reported. The symp-toms such as lumbago, sciatica and paresis of footwhich were perfectly cured at discharge relapsed afterseveral months of daily business. The two patientswere readmitted and reexamined by myelography andcomputer assisted tomography. In the two patients acystic pooling of metrizamide having a connection withthe subarachnoid space was noted in the same way.

At the second operation a small dural tear and anextradural arachnoid cyst were recognized similarly.
