側頭動脈炎—血管写上広範な血管炎を認めた1例

Involvements of Multiple Cranial Arteries in a Case of Temporal Arteritis Demonstrated by Arteriography

I．はじめに

　高齢者の頭痛の原因の1つとして側頭動脈炎は忘れてならないものであるが，わが国ではいまだ稀な疾患である^1）．この動脈炎は側頭動脈のみならず，他の動脈系をも侵すことが知られている．特に眼動脈系への炎症波及による失明は約1／3の症例に出現するといわれ^3），本症の重篤な合併症として知られている．本報告では側頭動脈炎の定型的臨床像と動脈生検所見を示した1症例を呈示し，広範な頭部の血管炎が頭蓋血管写上証明されたので，その意義について考察を試みた．

Temporal arteritis has been only infrequently reported in Japanese literature and so far only several pa thologically proven cases were found among these reports. Here, we report a classical example of temporal arteritis, in which the arteriographic demon-stration of arteritis was useful in order to choose an appropriate segment of arteries for biopsy.

This 81-year-old woman developed intractable headache and lumbago of one month duration prior to admission. She was found to have tender, tortuous,swollen temporal and occipital arteries without any neurological deficits.
