Although the cervical spondylosis produces a variety of neurological symptoms, there is pure motor syndrome which is difficult to be differentiated from motorsystem disease. Among 53 operated cases of cervical spondylotic radiculopathy and myelopathy, we encountered 7 cases classified as motor syndrome presenting predominantly muscular weakness without sensory deficits. Four of these cases showed amyotrophic lateral sclerosis-like symptoms such as finger muscle weakness, spastic gait and corticospinal tract sign while neck and shoulder pain on neck extension and paresthesias of the fingers suggested cervical spondylotic origin.
