I．はじめに

　全頸椎損傷中，上位頸椎損傷は約10％内外と報告されている^12）．hangman’s fracture^15）はJefferson’s frac-ture^8,11），歯突起骨折などの上位頸椎骨折の18-32％を占めるほど頻度の高いものである^9,13），しかし一般にこの型の骨折は脊髄に及ぼす影響が少ないため神経症状が軽微なことが多く^9,12,13），発見されないまま放置され，受傷後，数日から数週後に診断のつく例も少なくない^9，^14）．この骨折の発生機序について，絞首刑者にみられる骨折に類似することから^17,19），上位頸椎の過伸展が原因と考えられているが^15），過伸展だけでは説明のつかない例もある^4,6）．著者らは4例の本骨折を経験したので症例を検討し，受傷機転，神経症状，治療などについて考察を加える．

Four cases of hangman's fracture were reported. Thecases were distributed between 67 and 85 years of age andall were males.

The exact mechanisms of injuries to these cases were notprecisely documented, but all struck the head by falling forsome distance. Three of them showed cerebral concussionimmediately after injuries. One of four patients had com-plete transverse cord lesion at C5 level and the other showedright Brown-Séquard syndrome at C4 level, while anothertwo showed no neurological deficits except for neck pain.
