放射線治療後Alpha-fetoproteinが異常高値を示した頭蓋内原発Germ cell tumorの1例

Primary Intracranial Germ Cell Tumor with Abnormal High Value of Alpha-fetoprotein after the Radiation Therapy

I．はじめに

　最近alpha-fetoprotein（以下，AFP）やhumanchorionic gonadotropin（以下，HCG）などのtumormarkerに基づくgerm cell tumorの診断，治療の概念が，脳神経外科領域においても定着しつつある^6,14）．第3脳室近傍に原発するgerm cell tumorは，特に本邦の報告例が多く，さまざまな組織型のgerm cell tumorが報告され，tumor markerとの関連も詳しく検討されている．しかしgerm cell tumorでは，その組織型により，治療，特に放射線照射に対する反応が異なることから，治療により腫瘍の組織学的特徴がかなり変化する可能性が示唆される^15）．しかしながら，CT上の変化やtumor markerの変化により，実際にこれを確認した症例はいまだ報告がない．

　私どもは最近tumor markerとCT上の追跡により，踵瘍構成組織の変化ないしはAFP産生能の変化を示唆する1症例を経験したので，これを報告し，文献的考察を加える．

A 7-year-old boy was admitted to Hokkaido UniversityHospital complaining of headache and vomiting. On admis-sion he was slightly confused and presented Parinaud's sign.CT scan revealed abnormal high density mass with contrastenhancement effect at the pineal region and obstructivehydrocephalus. Laboratory studies showed the normal valueof human chorionic gonadotropin and no trace of alpha-fetoprotein. Germinoma was most suspected based on thefindings of CT scan and laboratory studies.
