I．はじめに

　閉塞性脳血管障害により惹起された急性期脳虚血の治療として，虚血部の循環改善，脳浮腫の軽減，神経細胞の虚血に対する抵抗性の増強などを目的とした種々の薬物療法が試みられている^30）．しかし，現在のところ十分に確立された治療法として評価するには至っていない．

　近年，脳虚血巣における進行性微小循環不全や血管内凝固異常の発生が報告されている^17,22,23）．これら微小循環障害がどの程度虚血の進行や神経症状の出現に関与するかは現在のところ不明であるが，prostacyclinなどの微小循環改善剤が急性期脳虚血に対して用いられ，ある程度の効果が認められている^{1,12,13,26）}．

Prostaglandin (PG) E₁ is a potent vasodilator on the peripheral vessels and also has an inhibitory action of platelet aggregation. Lipo PGE₁, the lipid emulsified PGE₁ has much longer half life time in the circulation than PGE₁ which is rapidly inactivated in the lung.

The purpose of this investigation was to study the clinical and hemodynamic effect of Lipo PGE₁ on the 15 patients with acute or subacute focal cerebral ische-mia. Of these patients, five had evidence of internal carotid artery occlusion, and ten had either occlusion or severe stenosis of middle cerebral artery.
