Ⅰ.はじめに
閉鎖性頭部外傷に起因する脳血管性障害のうち,内頸動脈の狭窄および閉塞については少なからず報告されている.しかし,その狭窄部位が改善されていく治癒過程を脳血管写で経時的に追跡し得た報告は少ない.
われわれは,頭部外傷後の脳血管写で内頸動脈に広範な狭窄が認められた症例を経験した.この症例に対し受傷後6ヵ月にわたり経過を観察したところ,血管写上狭窄部位の改善を証明し得た.本報告では,本症例を血管写と共に呈示し,さらに外傷後の脳血管写について若干の文献的考察を加えた.
This paper reports a rare case in which a development and improvement of post-traumatic stenosis of the internal cartoid artery was documented by angiographical stadies.
A 31-year-old male suffered from disturbance of consciousness with left hemiparesis following head trauma. On admission, plain craniograms showed fracture of the bilateral vertex and a fracture crossed the right carotid canal. Right carotid angiogram relealed that the internal carotid artery was visualized "striped" and its wall was irregular narrowing. Angiogram taken nine days after the trauma showed an improvement of the stenosis.
