トルコ鞍内に進展した神経鞘腫の1例

Neurinoma with Intrasellar Extension: A case report

I．はじめに

　最近われわれは，海綿静脈洞内に発生し，一部トルコ鞍内にも進展していたため，術前に下垂体腺腫の側方進展を思わせた神経鞘腫の1例を経験した，神経鞘腫の明らかなトルコ鞍内進展を確認した症例の報告は少なく^19），また発生母地に関して，本症例は海綿静脈洞部では最も一般的なganglion typeの三叉神経鞘腫より，むしろ動眼神経より発生した神経鞘腫が考えられたので，若干の文献的考察を加えて報告する．

A 64-year-old female visited Kawatetsu ChibaHospital complaining of left retro-orbital pain. Amonth before, she had experienced ptosis anddiplopia, that had been releaved two days later bycorticosteroid. On admission, she had no neurolo-gical deficits except for minimal anisocoria, with theleft pupil larger than the right. There was no cuta-neous manifestation of von Recklinghausen's disease. Skull X-ray films showed depression of the floor ofthe sella turcica on the left side. CT scans demon-strated a parasellar enhancing mass with intrasellarextension.
