I．はじめに

　中枢神経系への転移腫瘍のうち，脊髄髄内への転移は稀であり^{1,12,15,16）}，臨床的には転移性脊髄病変による症状が他臓器症状に先行し，臨床診断が困難な場合がある^7,17,19）．われわれは，上肢痛，四肢脱力等の脊髄症状にて発症し，臨床経過中確定診断が困難で，発症より6カ月後の剖検により脊髄髄内および他臓器への広範な転移を認めた肺癌の1症例を経験した．症例を報告し，脊髄髄内転移腫瘍の診断上の問題点につき考察を述べる．

An autopsy case of an intramedullary metastatic spinal cord tumor from lung cancer is reported.

This 65-year-old man developed bilateral shoulder pain on September 10, 1977 and the pain gradually progressed. On September 20, the pain became to extend to both hands, and motor weakness appeared in both upper limbs more on the left. At the end of October abnormal hot sensation appeared in the lateral side of the right lower limb, and the motor weakness extended to both lower limbs and was accompanied by difficulty of urination.
