新しい脳保護物質の実験的検討—マンニトール，フルオゾールによる高度虚血脳の機能回復

Experimental Study of New Cerebral Protective Substances:Functional recovery of severe, incomplete ischemic brain pretreated with mannitol and fluorocarbon emulsion

I．はじめに

　脳梗塞に対し種々の治療方法が試みられてきたが，更に有効な治療法の開発が待望されている．われわれはmannitolによる脳梗塞の発現予防効果について検討し^24,27,29），既に実際の脳動脈瘤手術の臨床応用の面でも報告してきた^{21,22,25,26）}．更に近年，人工血液として開発されたfluorocarbon emulsion（FC）の高い酸素運搬能^3）に着目し，FCの虚血能に対する防禦効果についても種々実験的に検討している．本報では，実験動物として最近当教室で独自に開発した，大脳半球の循環血流量を灌流ポンプにより自由に調節できる"完全虚血脳灌流モデル犬"^11）を用い，その脳波活動を指標として，これら薬剤を検討したところ，特にmannitolとFCを併用した場合に，優れた脳保護作用を示すことが判明したので報告する．

Using the "canine model of complete ischemic brain regulated with a perfusion method" in which it is possible to control the degree of blood flow to a cerebral hemisphere via a perfusion pump, the effects of mannitol (which acts as a free radical scavenger) and FC (which has 0.1μm of average particle size and a high oxygen-carrying capacity) on cerebral ischemia were investigated. After pretreatment with the drugs, blood flow was reduced via the pump to 1/10 of the normal flow volume and 1 hr later, return to a normal flow was allowed. Subsequent changes in electrical activity were observed and the effects of the drugs evaluated.
