脳動脈瘤直達手術後の精神症状の評価—とくにContingent negative variationの応用

Ⅰ．緒言

　脳動脈瘤の破裂ないしはその直達手術後に，Korsakoff症候群^{1，11，12，14，16，17）}，akinetic mutism^3），情動，性格の変化^{1，11，12，14，17，23）}ほどの精神症状が出現することがあると報告されている，しかし，その発生頻度は報告者により著しい差があり^{1，4，11，12，14，17，20，23）}，精神症状を発現せしめる要因についても，動脈瘤の位置，破裂の状況，発作の重篤度など手術前の状態^1，11），手術手技^{1，12，22）}が挙げられ，なお明確にされていない，また観察期間や診断時期^{1，11，12）}によっても精神症状の出現状況は異なっている．

　その最も大きな理由の1つは，精神症状のとらえ方と評価法に画一性を欠き，さらには健忘，性格変化，作業能力の低下など精神機能の低下ともいうべき状態の客観的評価法が確立していないことに求められる．

There are several reports that mental disturbance occurs following a some kind of operative manipulation or temporary clipping of the main artery during direct aneurysm operation. However, other reports deny it. It is controversial that an occurence of mental disturbance following direct operation for intracranial aneurysm is observed in all cases, because the clinical evaluating method for mental disturbance except conventional clinical psychological method is not quite established and even the cases having ruptured aneurysm without direct operation.
