くも膜下出血で発症した第3脳室内頭蓋咽頭腫の1例

A Case of Intraventricular Craniopharyngioma with Subarachnoid Hemorrhage

Ⅰ．はじめに

　頭蓋咽頭腫は稀に第3脳室内に限局して発育し^4-6,15），その臨床症状はもっぱら頭蓋内圧亢進の症状を呈し，視神経障害や下垂体一視床下部系のホルモン欠損症状を全く示さない．われわれはくも膜下出血で発症した頭蓋咽頭腫の剖検脳を検索したところ，腫瘍は第3脳室内に限局して発育しており，視床下部の脳組織内に腫瘍の発生母地を思わせる所見を見出したので，この稀な症例を若干の文献的考察を加えて報告する．

A 30-year-old man was admitted on syndrome of subarachnoid hemorrhage. Neurological findings disclosed only slight nuchal stiffness. Computerized tomography and pneumoencephalography revealed a mass in the third ventricle. By right frontal craniotomy the anterior horn of the lateral ventricle was opened. It was demonstrated that the Monro's foramen was dilated and obstructed by a tumor in the third ventricle. The tumor was removed partially through the foramen and ventriculo-peritoneal shunt was made.

The headache and nuchal pain subsided after the operation.
