髄液中に腫瘍細胞を証明した斜台脊索腫の1例

Intracranial Chordoma:Report of a Case with Positive Cerebrospinal Fluid Cytology

Ⅰ．はじめに

　頭蓋内脊索腫は，稀な脳腫瘍でその頻度は全脳腫瘍の1％に満たない^34）．一方髄液細胞診は，1891年Quinkeが腰椎穿刺法を紹介後，Ravautらにより始められ^4），その後採取法，集細胞法^8，19）および脳腫瘍患者よりの腫瘍細胞の検出などについて非常に多くの報告がある^{14，20，21，29）}．

　最近われわれは斜台部脊索腫症例の精査中，脳室写時に採取した髄液中に腫瘍細胞を検出した．われわれが収録しえた文献の限りにおいては，髄液中に腫瘍細胞を検出した頭蓋内脊索腫は，その報告を見ない．

Intracranial chordomas are very uncommon and rarely invade the dura mater. This is a report of a case with clivus chordoma, the cells of which were detected in the cerebrospinal fluid (CSF).

A 40-year-old house wife was hospitalized on May 31, 1976, complaining of progressively deteriorating level of consciousness for two months prior to admission.

Neurological examination on admission revealed marked reduction of her mental level, and fecal as well as urinary incontinentia.
