I．はじめに

　第3脳室腫瘍へのapproachは，Dandy以来，種々の方法が行われている（Fig．1）．すなわちDandyの右前頭葉切除Monro氏孔経由^3），Poppenの同部切開Monro氏孔経由^17），Van Wagenenの後頭葉切開経側脳室経由^26），Dandyのtrans-callosal松果体部腫瘍摘出^1,2），Jamiesonのsuboccipital trans tentorial approach^7），Steinのsubtentorial supracerebellar approach^21）など枚挙にいとまがない^{1-3,5,6,9,10,13-23,25-27）}．いずれにしろ，脳実質の損傷，特に第3脳室周辺の重要な脳の損傷がなく，また動静脈の犠牲を極力避ける方法で行われなければ，術後発生することが予想される重大な合併症を克服して摘出することはできない^4,6,12）．第3脳室内の腫瘍を摘出するためには，どうしてもその壁のどこかを損傷して入らなければならないが，その壁の最も薄い部分は，第3脳室前壁にあるlamina terminalisである．

In this paper the authors reported the operativeapproach to third ventricle tumors, mainly discussingbifrontal translamina terminalis approach.

Our principle for approaching to the third ventricletumors is as follows. If the tumor originates from theupper wall behind the Foramen of Monro, transcallosalapproach should be adopted. And if the posteriorwall is its origin, either transcallosal or infratentorialapproach would be better. But if the tumor arisesfrom the anterior half of the third ventricle, trans-lamina terminalis approach should be adopted.
