脳血管撮影により誘発された脳動脈瘤破裂—1症例と文献的考察

Rupture of Intracranial Aneurysm due to Cerebral Angiography

Ⅰ．はじめに

　脳血管撮影は，1927年Egas Monizによって，はじめて報告されて以来脳神経外科における最も重要な補助診断法となったが，とりわけ脳血管性病変の診断には欠くことができない．一方，種々の合併症が多数報告されているが，中でも最も危険なものに脳動脈瘤破裂がある．このものの発生頻度は極めて稀れであるが，事の重要性のため，報告は散見される．最近我々も，このような症例を経験したので報告するとともに，現在までの文献的考察を含め，その発生機序と問題点について考察した．

The authors report a case of rupture of intracranial aneurysm by angiography which was done four hours after the subarachnoid hemorrhage.

Case; A thirty-one year old male patient was brought to our outpatient's clinic by ambulance because of conscious loss and convulsive seizure on Feb. 5th, 1974. Lumbar puncture showed grossly hemorrhage in the CSF. Immediately he was hospitalized and administered anticonvulsants, hypotensive drugs, antifibrinolytic agents and corticosteroid.
