Brain Scintiphotogramm in Sarcoidosis of Central Nervous System: A Case Report

Ⅰ．はじめに

　Sarcoidosisは原因不明のgranulomatous変化を全身諸臓器に及ぼす系統的な疾患であるが，中枢神経系に病変を作ってくるものは5％以下とされている．この場合，その診断は臨床経過からつけられることが多いが，脳波や脳血管写で変化をみつけられることもときおりある．

　私共は，全身にわたる進行性sarcoidosisに加えて，視野欠損，尿崩症を来たして入院加療中の症例に，ガンマカメラによる脳シンチフォト像を撮影したところ，下垂体部周辺に境界不鮮明なRIのとり込み像を認めた．その後，この症例は不幸にして死亡し，剖検によって下垂体周辺より第三脳室内壁におよぶsarcoidosisの発症を認め，シンチグラムのRI集積の局所と病変部の一致することを確認した．

Involment of the central nervous system in sarcoidosis occurs in approximately less than 5% of the cases.

The authous reported cerebral sarcoidosis in a woman of 40 years old who had suffered from sarcoidosis since 1963. Seven to 8 years later she developed diabetes insipidus, visual field defect and obesity, which suggested strongly a pathological change around the pituitary gland.

This change was confirmed by brain scintiphotogramm used gammacamera which showed concentrated accumulation of 99 m-Tc after 9 years of her clinical course.
