系統変性症における自律神経障害の神経病理

Neuropathology of the autonomic nuclei in system degenerations.

I．はじめに

　神経原性の自律神経障害として発表されたShy-Drager症候群（SDS）には^35），少なくとも2つの問題があると思われる。その1つは臨床症状に対応する責任病巣である。SDSを特徴づける起立性低血圧は従来から胸髄側柱とされてきたが，最近では脊髄交感神経節も候補に挙がっている。これは臨床病理学的相関（clinicopathological correlation）の問題であり，神経病理学的には末梢を含めた自律神経系全体の見直しが必要であろう。

　第2の問題は，オリーブ橋小脳萎縮症（OPCA）や特発性パーキンソン病（iPD）で臨床的に自律神経症状が強かったり，臨床的にSDSと診断された症例が病理学的にはOPCAであったというような診断学的，疾病分類学的な混乱である。これは，Shy ＆ Dragerが報告した症例には自律神経症状のみではなくて，OPCAを示唆する症状と病理所見もあったことに端を発する。すなわち，それを契機にOPCAの自律神経障害について臨床的，病理学的に再検討されることになり，線条体黒質変性症（SND）とOPCAの関係を含めて概念の再編成にいたったが，それが診断あるいは疾病分類に少なからず混乱を引き起こしたのは事実である。

Neuropathological study was performed on the spinal cord and brainstem autonomic nuclei, and the hypothalamic nuclei of various system degenerations (see Tables 1 & 2). Pathological changes found in the autonomic nuclei in the brain were pronounced in idiopathic Parkinson disease (iPD), olivopontocerebellar atrophy of sporadic type (sOPCA, or multiple system atrophy, MSA), OPCA of hereditary type (hOPCA) and progressive supranuclear palsy (PSP), while the other diseases showed no obvious changes.
