先天代謝異常症のスクリーニング

Screening for inborn errors of metabolism

　先天代謝異常症の種類は，今でも少しずつ増加しており，この領域で有名な成書"Metabolic Basis of Inherited Diseases"でも^1），版を重ねる度に，扱われる疾患の数が増加している。これと並行して，治療可能な疾患も，徐々に増加している。これらは，多くは早期に治療を開始することが必要である。このため，先天代謝異常症の患者を早く発見する，スクリーニングの方法が，種種考案され，その一部は，行政的サービスとして広く普及しつつある。本章では，先天代謝異常に対する個々の研究室での分析方法ではなく，マススクリーニングの方法に限局して，概要をのべたい。

　先天代謝異常スクリーニングという場合，現在では生後1週以内に，後述のガスリー法により採血して，検査を行なう新生児スクリーニング（neonatal screening）が主であり，近年生後1か月以後，あるいは，何らかの症状が現われた時に行なう，乳児スクリーニングも研究されつつある。この2種類のものを分けて概観する。

1) Neonatal screening

　A portion of inborn errors of metabolism can be treated successfully and many of them need early initiation of the treatment to obtain an ideal results. Especially, diseases which may cause brain damages or early infantile death, must be treated as early as possible. In 1961, R. Guthrie, USA, established an excellent method to detect PKU babies, and field experiments of the screening for PKU was started in 1963 in many states of the USA, and many PKU patients were detected.
