定位脳手術による疼痛の治療

Stereotactic operation for relief of intractable pain

　今日頑痛の治療には外科的，非外科的を問わずいくたの方法があるが，一つの方式だけですべてのいたみの治療を完結できるものではない。いたみは他の知覚系とは異なり感覚系としては原始的であり，特殊感覚系のように末梢の受容器から大脳皮質までの感覚弁別性求心系のみで成立しているのではなくて，その伝達には中枢神経内での修飾，促進，抑制などが複雑に絡み合っており，いっぽうでは運動系や情緒系，その他の系とも密接な関係を持っている。したがって頑痛の治療というものは簡単ではない。ここでは疼痛の治療のうち，頭蓋内において定位脳手術法を用いて行なわれている主な治療法について述べる。

Abstract

Stereotactic supratentorial intracranial lesions which may effectively control chronic agonizing pain in man without somatic sensory disturbances. They include

1) operation of Truncothalamus (intralaminal nuclei)

2) operation of mesencephalic reticular system

3) operation of Palliothalamus (pulvinar)

4) operation of limbic system (anterior cingulum, amygdala and posteromedial hypothalamus)

5) hypophysectomy

6) deep brain stimulation (thalamic sensory relay nuclei, periventricular grey matter, periaqueductal grey matter……)
