脳下垂体アルコール注入による癌性疼痛の除痛

Some considerations of the mechanism on relief of cancer pain by pituitary neuroadenolysis

I．序　言

　内分泌依存性腫瘍の治療および転移巣の疼痛除去の目的では，ホルモン薬物療法や，外科的な卵巣，睾丸，副腎摘除のほかに脳下垂体剔除が行なわれている^1）。しかし手術侵襲が大きいため，¹⁹⁸Auや⁹⁰Yペレットの下垂体内留置による破壊術などが提唱されたが，効果発現に数日を要するなどの不確実性によって成功していない。近年では下垂体の高周波凝固法や冷凍手術も試みられている^2）。

　1964年，Moriccaは^3）経鼻経蝶骨洞下垂体アルコール注入法によって癌性疼痛が劇的に寛解することを発見した。この除痛はアルコール注入直後に起こることから，内分泌機能抑制が主体とは考え難く，非特異的除痛法として全世界で追試されるようになった^4）。

Abstract

Since 1977, we performed Prof. Moricca's neuroadenolysis 135 times on 101 patients. Out of these, 42 were suffering from hormone-dependent malignant neoplasms, and 59 patients were suf-fering from non-hormone-dependent ones. Out of a total of 135 times of the procedures, 95 times, that is about 70%, eventually obtained complete pain relief, but failure to obtain pain relief was noted in 7 cases. With hormone-depend-ent tumors, complete pain relief was obtained in 87%. In 59 non-hormone-dependent tumors, complete pain relief was obtained in 59%.
