Current surgical treatment of cerebrovascular occlusive disease in our department is reviewed. Its theoretical backgrounds are briefly enumerated from the following points of view ; 1. natural history of cerebral ischemia. 2. penumbra. 3. revascularization in the acute stage of ischemia. 4. cerebral edema due to infarction.

Type of operation, its numbers and mortality up to the end of July 1981 are listed in Table 1.

Carotid endarterectomy has been performed using operating microscope without the internal shunt with artificial hypertension during clamping of the ICA, monitoring stump pressure and power EEG.
