Lafora病の臨床と組織病理学的研究

Clinical and Histopathological Investigation of Lafora Disease

　Lafora病の人種問題と，臨床的経過および組織病理学的所見との関係を明らかにし，また，Lafora小体，心，肝の病変ならびにアミロイド小体の電顕的研究を行なつた。

（1）Lafora病の罹患に関し，日本とヨーロッパの間では人種差は無いようである。

（2）臨床的には，てんかん発作初発前の異常脳波と性格変化が重祝されねばならない。てんかん発作初発前に，脳内にLafora小体が形成されている可能性がある。

（3）脳波の病的変化は，臨床症状および組織病理学的変化とかなりよく平行して悪化進行する。

（4）弱Bouin液固定時のLafora小体内細線維は，ミエリン様像を示す。その大部分はBakerのピリジン抽出法で溶け去る。

（5）Lafora小体の形成には，グリコーゲン類似物質とともにphospholipiidsが関与している。

（6）Lafora小体の形成には，原形質膜とその外側面の物質が関与している可能性がある。

（7）Lafora小体とアミロイド小体間の密接な関係が暗示された。

（8）Lafora病の脳における局所的相違の基礎は，原形質膜とその外側をおおう物質に求められるかもしれない。

　The relation between the clinical probleme and the clinical course as well as the histopathological view is followingly clarified, together with the elctronmicroscopic investigation of Lafora body, and corpora amylacea, and on the pathological change of heart, liver, as well have been carried on at the sama time.

　1. There seems no difference of the rase in the sufferering of Lafora disease.

　2. The abnormal electroencephalogram and the decline of character before the first paroxysm of epileptic seizure clinically have to take the serious view of researches. There is the possibility of building up a Lafora body inside the brain fefore the first paroxysm.

　3. The change of electroencephalogram is comparatively parallel to the aggravation with the histopathological change.

　4. The filamentous structure inside Lafora body when used Bouin's fixative shows myelin-like figure, and the most parts of it melt away through Baker's pyridin extraction method.

　5. There is probably a relation of phospholipids in the formation of Lafora body.

　6. There is a possibility of participation of plasma membrane and intercellular material in the formation of Lafora body

　7. It is suggested that between, Lafora body and corpora amylacea a keen relation may exist.

　8. It is guessed that in the background of pathologic haemoarchitecture in the brain of Lafora disease we seek in the plasma membrane and in the intercellular meterials, as well.
