入院手術前に急死した脳腫瘍症例の臨床病理学的研究

Clinicopathological Studies on the Preoperative Sudden Death in Cases of Braintumor

I．はじめに

　脳腫瘍ないしはその疑診がおかれた患者のなかで，適切な治療を施し得る病院への輸送の途中とか，入院後の検査中に，急激な症状の増悪をきたし遂に開頭術に至らずして急死するという極めて不幸な症例がなかにはある。このような入院後手術前の急死例は決して多いものではなく，我々の教室では脳腫瘍562例中わずか25例（4.4％）にすぎない。しかしこれはあくまでも専門医のいる病院の統計であるから，一般にはこれ以上の数値に達するものと思われる。しかもこれら急死例は適切な処置を施せば治癒の可能性があつたものが多いだけに，その発現頻度がそう高くないからといつて無関心でいることはできない。そこで，我々の経験した術前急死例25例の臨床経過と病理解剖所見を検討して，25症例の死因を考察する。

The cause of preoperative sudden death in braintumor were studied clinicopathologically on 25cases admitted in the department of Niigata Unive-rsity. Almost all of these cases died showing signof brain herniation after manifestation of increasedintracranial pressure. Pathological study revealedbrain herniation in all except four special cases.And long survivers showed hemorrhage, edemaand ischemic changes of the nerve cell at the brain-stem. These histlogical findings support the signof brain herniation in the clinical course.

The direct cause of the sudden death was revealedto be due to the vascular lesion at the brainstemresulting from brain herniation. The moment cau-sing brain herniation was discussed.
