広汎な大脳傷害例にみられた無動，無言状態について

Akinetic-mute states on cases of the widespread cerebral degeneration

Ⅰ．まえがき

　覚醒しているようにみえながら，自発的にも刺激に対しても，動きや発語が失われ，全く疎通性の得られない無動無言状態は，個々の精神神経機能よりはむしろ，その背景に流れ動く存在の全体的変化，あるいは内外刺激の認識統合系路の障害ともいえるが，神経学や神経生理学の面から，意識の機構や局在の研究が進められ，中脳網様体および間脳—大脳皮質系（網様賦活系）とが，それぞれ覚醒と統合に関与する意識の荷い手として重視されるに及んで，無動性無言症もこれらの部分の障害と関係づけて考えられるに至った。しかしながら実際面では，ひとくちに無動性無言症といっても，状態像は個々例に依存した多様性を示し，傷害の局在による症状の特徴や，こうした状態の基本障害について，すべてを充分納得させてはいない。

1. Awaking but akinetic-mute states in two cases of the widespread cerebral degeneration were presented (Case 1 and 2).

2. In Case 1 (young adult type of the hepatocerebral disease) there was shown an akinetic-mute state with spastic paralysis, extended posture and some released primitive phenomena (decortication syndrome) in reference to severe destruction of the cerebral hemispheres.
