頸椎後縦靱帯骨化症の1剖検例

An autopy case of ossification of the posterior longitudinal ligament of the cervical spine

　頸椎の後縦靱帯骨化は1960年月本^1）の報告以来，神経学領域においても次第に注目を集めている。彼は生前に特異なX線像を呈した症例の頸椎を剖検により検索し，頸部後縦靱帯の化骨を明らかにしたが，1964年に至り，寺山ら^2）は剖検例を含む6例の詳細な検討から，既知の脊椎変性疾患のいずれにも属さない特殊な病態と解釈し，これを後縦靱帯骨化症と呼んだ。以来，数多くの症例報告を重ねる中で，本疾患はますますclinical entityとしての座を明確にして来ているが，その剖検報告は少なく^1〜5），いまだその臨床病理学的研究は十分でない。今回，著者らは本症の1剖検例を検索する機会を得，後縦靱帯骨化の病理像ならびに，それによる特異な脊髄変性につき2,3の知見を得たのでここに報告する。

An autopsy case of 71 year-old male with os-sification of the posterior longitudinal ligament (OPLL) of the cervical spine was reported. The cervical spine was resected en bloc with the spinal cord. The radiological and morphological exami-nations revealed the following: 1) An end of the rentogenologically revealed ossification was always fused with the posterior edge of the vertebral body. The histological exam-ination of this part showed both the degenerated disc protrusion and increase of thickness and com-pact of vertebral body edge.
