はじめに

　特発性起立性低血圧症はShyおよびDrager^27）により臨床ならびに病理学的所見が記載され，以来外国でも100例以上の本症例が報告された。本症では起立時における血管運動反射路が神経系の種々の部位で傷害されているが，この場合，求心路，中枢路および遠心路のいずれかの障害によると思われた。また散発的に起ることが多いが，まれに家族的にみられることもある（Lewis症例^18））。剖検所見も報告されており，その中には明らかな神経症状があるにもかかわらず病理学的にはほとんど変化を示さないこともあり（Martinらの症例^19）），また臨床的にはほとんど神経症状がみられなかったもので，剖検で中枢神経系に明らかな病的所見を示したものもある（Vanderhaeghenらの症例^31））。

　私どもは特発性低血圧症について私どもが経験した2,3の症例を述べるとともに，本症の総説を試みることにする。

A review was made on idiopathic orthostatic hypotension in association with Shy-Drager syn-drome. Clinical symptoms, pathological findings, classification of the disease, identification of impair-ed loci, and pathophysiology have been described. The review also dealt with the mechanism of an alternation of Horner's syndrome from side to side that uncommonly occurs and the patients' response to medication and their susceptibility to nicotine.
