The circadian rhythm of blood pressure was inves-tigated in a patient with Shy-Drager syndrome and in 6 normotensive controls using an ambulatory blood pres-sure monitoring system.

In the patient with Shy-Drager syndrome, sleeping-state blood pressure was significantly higher than that in the awakened-state. It was also observed that there tended to be a negative correlation between blood pressure and heart rate. These results were clearly different from those in normal controls. Thus, it was suggested that a Shy-Drager syndrome patient has aspecific circadian rhythm of blood pressure caused by the dysfunction of the autonomic nervous system, and that ambulatory blood pressure monitoring is very useful to reveal the characteristics of the daily blood pressure change in Shy-Drager syndrome.
