Shy-Drager症候群患者1例の血圧日内変動を携帯型自動血圧測定装置を用いて,正常血圧者のそれと検討した.Shy-Drager症候群患者では,睡眠時の血圧は覚醒時の血圧より有意に高く,血圧と心拍数に逆相関の傾向がみられた.これらの結果は正常血圧者のそれと明らかに異なるものであった.Shy-Drager症候群では,自律神経機能障害による特異的血圧日内変動がみられ,本症の血圧変動の特徴を明らかにするうえで,携帯型自動血圧測定装置による血圧モニタリングは有用と考えられた.
The circadian rhythm of blood pressure was inves-tigated in a patient with Shy-Drager syndrome and in 6 normotensive controls using an ambulatory blood pres-sure monitoring system.
In the patient with Shy-Drager syndrome, sleeping-state blood pressure was significantly higher than that in the awakened-state. It was also observed that there tended to be a negative correlation between blood pressure and heart rate. These results were clearly different from those in normal controls. Thus, it was suggested that a Shy-Drager syndrome patient has aspecific circadian rhythm of blood pressure caused by the dysfunction of the autonomic nervous system, and that ambulatory blood pressure monitoring is very useful to reveal the characteristics of the daily blood pressure change in Shy-Drager syndrome.
