担癌例にみられるNecrotizing Myelopathyの臨床および病理形態学的再検討

Clinical and neuropathological investigation of necrotizing myelopathy associated with visceral carcinoma

I．はじめに

　最近，癌患者における"転移によらない"神経系病変，いわゆるcarcinomatous neuropathyの存在が問題にされている。1965年L. Brainら^5）はそうした病変をnon-metastatic carcinomatous neurologic diseasesとして分類を試みた（Table 1）。その分類の中で，myelopathyの項に含まれ記載されているsubacute necrotic myelopathyは1964年Mancallら^22）がnecrotizing myelopathy associated with visceral carcinoma（以下本症と記す）として報告したものから引用されたものである。Mancallらは2自験例と9文献例を検討し，その特異な脊髄病変を説明しうる有意な癌転移を認め得ないことから，癌による代謝障害の可能性を推測し，非常に稀な疾患であるとした。その後，剖検例の追加は見あたらない。

　われわれは本症と癌転移の関係を再検討する目的から，Mancallらの症例と，われわれが剖検後最終的には癌転移も認め得た脊髄病変例の臨床的および病理形態学的類似性を検討した。

L. Brain et al. (1965) discussed the non-metastatic carcinomatous neurologic diseases, including subacute necrotic myelopathy of which two cases had been reported by E.L. Mancall et al. (1964) and additional 9 cases found in the past literature. They could not detect significant metastasis of neoplasm, and suggested that the changes of the spinal cord might be due to the deranged metabolic processes somehow being related to the neoplasm elsewhere in the body.
