Morquio病の1剖検例，とくに中枢神経系蓄積症の新知見について

An autopsy case of Morquio's disease, with special reference to a new finding of neuronal storage in the central nervous system

はじめに

　Morquio病は1909年にVoisin^1）によりはじめて報告され，1929年にMorquio^3）とBraisford^2）とにより別々にその詳細な臨床症状が発表された。

　1961年Zellwegerは白血球にReilly bodyと尿中acid mucopolysaccharide（以後AMPS）の増加を報告し，1962年Pedriniは尿中増加AMPSはkeratosulfateが主であることを指摘した。1965年にMckusik^8）はmucopolysaccharidosisの概念を確立しこれを6型に分類し，本症を第4型に位置づけ，尿中増加AMPSはkeratosulfateで，遺伝形式は常染色体劣性，臨床特徴として重症な骨障害などをあげ，通常知能は正常とした。しかし1967年Matsaniotisは白血球中にReilly bodyを認めるが，尿中AMPSは正常で，骨格系の異常のみを示した本症の2例を報告している。以上のように本症は臨床症状からもまたkeratosulfateを中心とするAMPS排泄増に関してもいまだ一定の見解は得られていない。

　A 12 year-old boy clinically diagnosed as Morquio's disease with mental retardation was autopsied and pathologically examined. In the skeletal system besides osteochondrodystrophy characteristic of Morquio's disease there were aggregations of foamy cells in the ossifing layer and the perichondrium.

　In the central nervous system degeneration of the ascending and descending tracts of the spinal cord was observed and neurons of the cerebrum contained a large amount of lipofuscin-like granules.
