皮質形成異常の免疫組織学的研究―手術摘出標本による検索

Immunocytochemical study of focal cortical dysplasia: Investigation of surgically resected specimens.

Focal cortical dysplasia（FCD）は発生異常に基づく多様な組織像を示すとともに強いてんかん原性を有する。てんかん患者の手術摘出標本を用いて易興奮性に関連する免疫組織化学的変化を中心に検索した。その結果，GABA_A受容体およびグルタミン酸脱炭酸酵素（GAD）陽性の神経線維や細胞体が形成異常の程度に応じて減少し抑制系の障害が示されたのに対し，興奮性のグルタミン酸受容体（GluR-1，GluR-2/3，NMDAR-1）は有意な変化に乏しかった。一方，FCDに特徴的な形態異常を伴う大型の神経細胞には幼若期の細胞骨格蛋白が強く発現し，未成熟な神経細胞であることが示唆されたが，GABA_A受容体やGADに比べグルタミン酸受容体に好染していた。このようにFCDに認められる興奮系と抑制系の不均衡がてんかん原性と密接に関連していると考えられる。

Focal cortical dysplasia (FCD) as a cause of intractable partial epilepsy had a good surgical outcome. Its pathological features were disruption of cortical lamination and morphologically abnormal neuroglial cells with some degree of abnormal maturation and differentiation. Recently, intrinsic epileptogenicity of FCD has been shown by intraoperative corticography and the drug induced seizure-like discharges in slices of lesions.
