小児てんかんのPET研究

PET study of childhood epilepsies.

てんかんは機能的疾患であるので，機能的画像診断法であるpositron emission tomography（PET）は，てんかんの診断と治療に有用である。MRIなどの画像診断で構造異常が検出できない時でも，てんかん原性焦点やてんかん脳における機能的異常に関して有用な情報を提供してくれる。PETは他の機能的画像診断に比し解像力に優れており，難治てんかんの術前検査としてばかりでなく病態生理の解明にも有用である。West症候群やLennox-Gastaut症候群のように全般てんかんに分類されているてんかんにおいてもしばしば焦点性低代謝域が検出され，これは臨床経過に関連した消長を示すことから，てんかん原性焦点の機能的状態と関連があると考えられる。

Epileptic seizures represent functional alteration of the brain. To evaluate functional abnormalities of the epileptic brain, PET provides useful information on the metabolic alteration of epileptic disorders, supplementing EEG assessing neurophysiological changes. In partial epilepsy, interictal PET usually shows a localized area of hypometabolism in an epileptogenic cortex, but the area of hypometabolism usually extends beyond the epileptogenic focus.
