MEGによるてんかん原性域診断

Magnetoencephalographic diagnosis on the epileptogenic region.

MEGによるてんかん原性域診断についての研究の歴史的展開を概観した。MEGのてんかん臨床への応用は，これまでもっぱらMEGに検出されたてんかん性発射（磁気棘）の単一双極子近似による発生源の局在化によってなされてきたが，この方法には理論上・技術上のさまざまな問題が存在する。これまでにMEGを用いた臨床的研究の対象としては，側頭葉てんかん，皮質性ミオクローヌス・EPC，小児良性ローランドてんかん，Landau-Kleffner症候群，および全般てんかんが採り上げられてきた。そしてとりわけてんかん外科への応用が探求されてきたが，発作時記録に困難があることなどから，MEGのてんかん外科に対する有用性は今なお確立されていない。最後に筆者らによる皮質病変のてんかん原性とMEG所見の関係に関する新しい研究を紹介し，MEGによるてんかん原性域診断の可能性と限界を展望した。

Development of magnetoencephalographic study on epilepsy was reviewed historically. Clinical application of magnetoencephalography (MEG) in epileptology has been almost exclusively dependent on the method of localization of spike generator by means of single dipole fit. There are, however, various problems in this method. One of them is from neurophysiological point of view, namely, intracortical propagation of spike activity and consequential inadequacy of the single dipole fitting. Another is the apparent asynchrony of MEG spikes and corresponding EEG spikes which makes it difficult to identify generators of them.
