家族性ALSにおいて，1993年以来第21番染色体長腕上のCu／ZnSOD遺伝子に点突然変異が続々と見出され，ALSの原因としての活性酸素種の役割が注目されている。しかし発病メカニズムは単純なSOD酵素活性低下では説明できない。SODには，いわゆるSOD活性の他にperoxidase活性や銅毒性緩衝作用が知られているが，発病メカニズムと関連した具体的な活性酸素種はまだ特定されていない。一方peroxynitriteによる障害仮説も重要であり，ALSで見られるneurofilamentの異常との関連で注目されている。また運動ニューロンのアポトーシス過程におけるCu／ZnSODの遺伝子変異とミトコンドリア障害の関与の重要性もクローズアップされてきている。

The role of free radicals is an important aspect in the molecular mechanism of selective motor neuron death in amyotrophic lateral sclerosis (ALS). After the discovery of Cu/Zn superoxide dismutase (SOD) gene mutations in familial ALS in 1993, clinical severity and the SOD activity was extensively investigated. However, no cor-relation between them was observed, and the results suggest an alternative mechanism other than simple free radical damage by the reduction of SOD activity in the mutant SOD. A specific active oxygen species in the crucial mechanism of motor neuron death has not yet been identified.
