　中枢神経系は免疫学的特権部位と考えられてきたが，自己免疫性脳脊髄炎（EAE）や多発性硬化症（MS）など，サイトカインを介した免疫性機序による病態が存在することが明らかになってきた。EAEやMSでは炎症性サイトカイン（IL-1，IL-2，IL-6，IL-12，TNF-α，IFN-γなど）が病初期から優位に働き，一方，抑制性サイトカイン（IL-4，IL-10，IL-13，TGF-β）は極期や回復期に優位に働き炎症を収束へと導く。サイトカインは単独で作用するだけでなく，別のサイトカインの産生を誘導するなどネットワークを形成し免疫性神経疾患の病態に関与している。このような中枢神経系でのサイトカインネットワークの解明は，免疫性神経疾患の治療への応用の可能性があり，今後の発展が期待される。

The central nervous system (CNS) lacks lymphatic vessels and is protected from certain toxic substances by the blood-brain barrier. Based on these observations, the CNS has been considered to be an immunologically privileged site. However, recent studies on autoimmune diseases such as experimental autoimmune encephalomyelitis (EAE) and multiple sclerosis (MS) have shown that immune reactions mediated by cytokines take place in the CNS. Administration of proinflammatory cytokines or block of anti-inflammatory cytokines by neutralizing antibodies exacerbates the clinical course of EAE.
