筋移行による麻痺肢の機能再建

Functional restoration of the paralytic hand with reinnervated free muscle transfer

はじめに

　マイクロサージャリーの応用により遊離筋肉移行術reinnervated free muscle transferは麻痺上肢に高度の機能獲得を可能にした。現在，この筋肉移行術は腕神経叢損傷^1-6），未梢神経損傷，フォルクマン拘縮^7-9），四肢軟部悪性腫瘍切除後運動機能喪失^10）などの再建術として採用され画期的な優れた治療成績を得るようになった。本稿では筆者の遊離筋肉移行術の臨床経験をもとに麻痺上肢機能再建の現況，とくに腕神経叢損傷の再建に関してup-to-dateな話題について紹介する。

The author uses free muscle transfers for reconstruction of the upper limb to resolve the difficult problems in partial (upper-type or lower-type paralysis) and complete avulsion of the brachial plexus. In this article, the author's updated technique, the “double-free-muscle procedure” for total paralysis and the supplementary free-muscle procedure are described.
