神経移行術による麻痺肢の機能再建

Restoration of the upper extremity functions using nerve transfer

　神経移行術は末梢神経損傷に対する神経修復術の一つで，神経近位断端の状態が不良で神経を直接縫合することや神経移植により修復できない場合，神経を切断してもその機能は他の神経により代償できる神経，または修復を要する神経よりも重要度の低い神経を用い，修復を要する神経に移行して機能回復をはかる手術法である。現在行われている神経移行術は腕神経叢損傷節前損傷や分娩麻痺に対する種々の神経移行術，分娩麻痺交差過誤神経支配に対する肋間神経移行術，非修復性の正中神経損傷に対する橈骨神経浅枝の移行による知覚再建，顔面神経麻痺に対する舌下神経移行術などがある。ここでは腕神経叢節前損傷と分娩麻痺の交差過誤神経支配に対する機能再建術としての神経移行術について述べる。

Nerve transfer is the procedure to restore motor and sensory functions for irreparable nerve injuries such as the preganglionic type of brachial plexus injury, obstetrical paralysis and facial nerve paralysis. Procedures of nerve transfer and their results for brachial plexus injury and obstetrical paralysis with cross innervation are reported.

The donor nerves available for nerve transfer after brachial plexus injury are the intercostal nerve, the spinal accessory nerve, the cervical plexus, the phrenic nerve, the contralateral C7 root and the ulnar nerve.
