The purpose of this paper is to discuss current state of imaging diagnosis of upper spinal trauma. Clinical importance of plain film, high resolutional CT, and MR imaging. And then discussion of mechanism of trauma was taken place. Mechanism is categorized 4 groups. The most frequent mechanism is flexion and compression injury, which estimates 85% in all spinal traumas. Second is extension injury. Third is rotation injury, and the last is shearing injury. Level and type of the injury are determined by the mechanism. Next “ABCS” approach of spinal trauma which is proposed by Daffner is discussed specially emphasis of importance of soft tissue abnormalities.
