I．一般外傷の画像診断

　従来脊椎外傷の画像診断は単純撮影，断層撮影，CT，およびMyelographyにより行われていた。MRIの出現により非侵襲的に脊髄の外傷性変化の描出も可能となったことから，脊椎損傷に対する画像診断は画期的進歩を遂げ，臨床の画像診断への依存度は増大してきている。しかし単純撮影は今でも第一選択の検査法であることに変わりはなく，その有用性および限界について知る必要があると思われる。ここでは単純撮影の適応，単純撮影の限界，GTの役割，MRI，の有用性について述べ，さらに損傷のメカニズム，および単純撮影の基本的読影法（ABCSアプローチ）について述べる。

The purpose of this paper is to discuss current state of imaging diagnosis of upper spinal trauma. Clinical importance of plain film, high resolutional CT, and MR imaging. And then discussion of mechanism of trauma was taken place. Mechanism is categorized 4 groups. The most frequent mechanism is flexion and compression injury, which estimates 85% in all spinal traumas. Second is extension injury. Third is rotation injury, and the last is shearing injury. Level and type of the injury are determined by the mechanism. Next “ABCS” approach of spinal trauma which is proposed by Daffner is discussed specially emphasis of importance of soft tissue abnormalities.
