高齢者の歩行障害の特徴

Characteristics of gait disorders in the elderly.

はじめに

　筆者らの神経内科の外来を受診する患者のうち，歩行障害を主訴とする人は非常に多い。老年者の日常の診療でしばしば感ずることは，明らかな運動麻痺や小脳失調症，あるいは深部感覚障害が認められないにもかかわらず，転倒しやすいことと，何となく歩行がおぼつかないことである。このようなバランス障害ともいうべき状態は，どのような理由で，どこが障害されれば生ずるのか，まだ十分解明されていない。また，老年者では小歩やすくみ足もしばしばみられる。ここでは，高齢者の歩行障害の一般を述べるとともに，老年者に特徴的なバランス障害，小刻み歩行，すくみ足についても考察したい。

Sway while standing increases with advancing age¹⁾. Postural support responses are slowed in the older subjects²⁾. Postural responses to the shift are disturbed earlier than the disorders of steadiness on standing⁶⁾. The measurement of standing time on one-foot is useful for the evaluation of postural regula-tion in the elderly⁶⁾. Two-thirds of the patients aged over 60 years had severe or moderate impairment of postural reflex⁷⁾. This suggests that postural reflex dysfunction may be an important cause of unexplained falls in the elderly. Cerebral dopaminergic mechanisms may decline with aging^10~12). Postural reflex impairment in the elderly may be partly resulted from declining striatal dopaminergic mechanisms.
