 The 20K human GH(20K hGH)is a naturally occuring isoform lacking residues 32-46 of 22K hGH. The 20K hGH binds to hGH receptors with equal affinity, but binds to GH binding protein(GHBP)with lower affinity compared to 22K hGH. The 20K hGH isoform, when determined by a specific ELISA, comprises 6-7%of circulating immunoreactive hGH. No correlation was detected between the ratio of 20K hGH and age, body height, body weight, or body fat mass in normal subjects. The proportion of 20K hGH was significantly higher in patients with active acromegaly and patients with anorexia nervosa. The 20K hGH/(20K+22K hGH)ratio did not change after acute GH provocative tests, such as the insulin tolerance test and the GHRH test, suggesting that secretion of 20K hGH from the pituitary is under the same control as that of 22K hGH. The growth promoting activity of 20K hGH was similar to that for 22K hGH in rats. The 20K hGH dose-dependently increased serum IGF-1 levels and stimulated lypolysis with similar potency to 20K hGH in normal subjects and patients with adult GH deficiency. Thus, 20K hGH could be used for treatment with GH.
