頸部頸動脈狭窄症に対する血管内治療

The carotid angioplasty with stent

　はじめに

　高度頸部内頸動脈狭窄症は，頸動脈内膜剝離術（carotid endarterectomy：CEA）で治療されている。CEAはNASCETで症候性，70％以上の狭窄例，ACASで無症候性，60％以上の狭窄例において内科的治療にまさる成績が証明されている^{3, 4, 9）}。一方，CEAを行うには危険性の高い群も指摘されている。これら高危険度群に対して，CEAの代替治療として頸動脈ステント留置術（carotid angioplasty with stent：CAS）が行われるようになってきた。最近ではデバイスや技術の向上に伴い，より安全で非侵襲的なCASが増加傾向にある。今回は最近のステント治療の実際と，今後の動向・課題などについて述べる。

　This article is a review of the contemporary angioplasty with stent（CAS）. The indication of CAS, preoperative examinations, distal embolism, postoperative hypotension, hyperperfusion syndrome and future aspects of CAS were included.
