前立腺超音波像と生検像の比較

COMPARISON BETWEEN ULTRASOUNDGRAPHIC VIEW AND HISTOLOGICAL FINDING OF PROSTATE

　いす型ラジアル走査で前立腺に異常像を有した21例に，ピストル型ラジアル走査をガイドとして，経会陰的前立腺生検を試みた．この方法で目的部位の組織は確実に採取された．2例において，石灰化や澱粉様小体が，超音波上は高エコー領域として描出されることが確認された．また低エコー領域17例のうち12例は癌以外の組織であり低エコー像が必ずしも癌に特異的とはいえなかった．また，被膜の断裂としてとらえられる所見が，実際には前立腺の外の像をとらえていることもある所見が得られた．

Under the guide of pistol type radial scan, transperineal prostatic needle biopsy was perfomed in 21 cases.In 2 cases corpora amylacea was hyperechoic as well as calcification. Hypoechoic lesion was detected in 17 cases in which only 5 had cancer, thus hypoechoic lesion was not always specific for cancer. In 3 cases irregularities of the capsule were recoginized without cancer. We speculate in some cases normal components of extraprostatic tissue may be seen as capsular irregularity caused by cancer invasion.
